Request to change traveller information

If you wish to update your contact information after entering Canada, use the form below. You can also use it to notify us that you’re leaving Canada.

If you’re entering Canada, you may be required to quarantine or isolate in a suitable location with access to water, food, medication and heat. This location must be approved by a border official from the Government of Canada. You may leave this location only for a medical emergency, an essential medical service or treatment, or if it was pre-approved.

You must have approval from the Public Health Agency of Canada before changing your quarantine/isolation location. You may notify us after changing locations only in rare circumstances such as a fire or other immediate risk to your health and safety.

If you‘re submitting information for other travellers, complete a new form for each one. Start with your own information. The form will prompt you after each submission to add a new traveller. You must attest on behalf of each traveller.
